JustPhysio are looking to recruit a band 6 Discharge Planning Physiotherapist for a locum position with our client in London.

The pay for this position is £22 - £26 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.

Job Description:
Provide a wide range of outstanding care to patients within Newham who have been referred to the Referral and Assessment Team (RAT).
To provide meaningful choices that maximise health and wellbeing aiming to promote, as much as possible, an independent life.
To practice effective case management, assessing, planning, the implementation and evaluation of care including discharge. You will do this conjunction with the Professional Lead.
To be responsible for the care provided to the patients on the RAT case load and for the care provided by the team.
Ensure that a high quality service is provided to patients' in their homes, or residential care settings using available resources.
The service is provided from 08:00 till 20:00 for nurses and 09:00 till 17:00 for AHPs, 7 days a week.
Be responsible within the team for the liaison with the wider Newham MDT.

It is essential for applicants to have a minimum of 1-year post-graduate experience, right to work in the UK and HCPC registration.
