Job Description - Community Physiotherapist
Salary: £35,000 - £40,000 (dependent on experience)
Contract: Full-time
Location: Berkshire
Reports to: Clinical Director
About Our client -
Our client is a growing, multidisciplinary health clinic dedicated to helping people move better, recover faster, and stay active. They provide high-quality physiotherapy and a range of specialist services, including podiatry, women's health, Pilates, sports massage, and personal training. They are now looking for a motivated and compassionate Community Physiotherapist to join their team and deliver physiotherapy services directly within the community and for people being discharged from Hospital.
Role Purpose-
To provide high-quality holistic assessment, treatment, and rehabilitation for clients in their own homes, care homes or community settings, ensuring care is evidence-based, patient-centered, and aligned with the values of our client This includes working and liaising with carers, determining care levels are needed and working closely with local hospitals and other allied health care professionals.
Key Responsibilities-
* Undertake comprehensive physiotherapy assessments of clients in community settings.
* Develop and deliver individualised treatment and rehabilitation programmes.
* Provide care for a wide range of conditions, including musculoskeletal, neurological, elderly/falls, and post-operative rehabilitation.
* Work closely with families, carers, and other health professionals to support safe and effective care.
* Document all interventions accurately and in line with HCPC and CSP standards.
* Contribute to service development by identifying opportunities to enhance community-based care.
* Promote independence, self-management, and long-term health and wellbeing for all clients.
* Ensure safeguarding, confidentiality, and risk management procedures are adhered to at all times.
Requirements-
Essential
* BSc (Hons) Physiotherapy (or equivalent).
* Current HCPC registration and CSP membership.
* Strong clinical skills across MSK, elderly care, and rehabilitation.
* Experience of working autonomously in community or domiciliary settings.
* Excellent communication, time management, and organisational skills.
* Full UK driving licence and access to own vehicle.
Desirable
* Experience working with care homes or multidisciplinary teams.
* Knowledge of community health pathways and services.
* Pilates training, or willingness to undertake further professional development.
