Role/Job Purpose
* To set up and provide specialist Physiotherapy services with input into planning, development and evaluation of clinical services including documentation, assessment planning and tools.
* To assess and treat own caseload of patients and to maintain own records as an autonomous practitioner
* To manage a varied caseload of clients including Acquired brain injury and complex needs, orthopaedic rehabilitation and general rehabilitation using evidence based, client centred principles to assess, plan, implement and evaluate interventions.
* To assess equipment needs, maintain and ensure safe use within the team.
* To provide training when required to non-qualified members staff within the therapy team and other disciplines
* To work with the Manager and senior Physiotherapist, to be responsible for ensuring the safe, smooth and efficient running of the home in accordance with the policies of the home and the Care Quality Commission guidelines.
* To be professional and legally responsible and accountable for all aspects of own work, including the management of patients in your care.
Communication / Relationship Skills
* To communicate and liaise effectively with other members of the multi-professional team about patient issues in order to facilitate a team approach to patient care.
* To communicate effectively with clients, family members, and carers to maximise rehabilitation potential and to ensure understanding of condition.
* To use communication skills of persuasion, motivation, explanation and gaining informed consent with a wide variety of patients and recognise barriers to effective communication
* To provide planned advice, teaching, and instruction to relatives, carers and other professionals.
* To promote understanding of the aims of physiotherapy and to ensure a co-ordinated approach to patient care.
* To communicate effectively, both verbally and in writing, with members of the multidisciplinary team, other health workers and agencies involved in client care
* To work with the Manager and multidisciplinary team to identify and establish a client focused service building a good reputation with local commissioners and clinics
Quality
* To maintain a high standard of clinical care for the patients under your management.
* To maintain comprehensive and accurate patient records in line with legal, professional and departmental requirements.
* To maintain own professional development - through reflective practice, keeping up to date with new evidence and effective ways of working, assessing and treating patients, and changing practice in light of this as appropriate
* To attend appropriate post-graduate courses.
* To adhere to rules of professional conduct and facilitate compliance of rehabilitation assistance and other junior staff members with local and professional standards of practice
Client Care Responsibilities
* To formulate an individual management and/or treatment programme, using clinical reasoning skills and a range of treatment skills.
* To evaluate patient progress, reassess, and alter treatment programmes if required.
* To accept clinical responsibility for a designated caseload of patients, and to organise this efficiently and effectively with regard to clinical priorities and use of time.
* To ensure all aspects of care meet with agreed national, professional and local standards.
* To provide effective neurorehabilitation, long term disability management and slow stream rehabilitation to those with acquired brain injury under the supervision of a senior therapist
* To appropriate utilise SMART patient centered goals and outcome measures to facilitate, where appropriate, a safe and timely discharge into the community
* When required to provide Physiotherapy intervention in client's own homes following lone worker guidelines
