A unique opportunity has arisen to work in a modern, well equipped, multidisciplinary environment for people of working age with complex care, rehabilitation and/or life limiting physical and neurological conditions.
This permanent role is within a private rehabilitation care home based in Nottinghamshire.
You will be required-
To work within a therapy team, as a multi-disciplinary team including physiotherapists, occupational therapists, speech and language therapists, a therapy clerk and therapy assistants. To provide high quality patient-centred care through assessment, diagnosis, development and delivery of individual treatment programmes. To take the responsibility for own caseload, within a defined area of service or for a particular patient type. To provide assistance in the supervision, support and education of unqualified staff and students. To participate in departmental audit in support of evidence based practice.
* To undertake clinical assessment, including risk analysis, of patients presenting simple and complex pathologies over a wide range of specialties.
* To use basic clinical reasoning skills and manual assessment techniques to establish accurate diagnosis.
* To use clinical judgement skills and comprehensive knowledge of physiotherapy treatments to formulate individual treatment programmes based on evidence of best practice and continually review these as appropriate.
* To demonstrate skill and expertise in the delivery of treatments to patients by interpretation of sensory feedback and the use of handling, manual physiotherapy techniques, equipment, assistive devices, respiratory clearance techniques and exercise therapy.
* To regularly evaluate the effectiveness of the treatment plan and adjust the treatment accordingly.
* To use clinical knowledge to formulate a prognosis and recommend an appropriate course of action and discharge plan.
* To maintain comprehensive, accurate and contemporaneous treatment records including valid consent appropriate to the intervention.
* To ensure that patient confidentiality is maintained at all times.
* To support on therapy outings for hydrotherapy, clinical appointments, community outings and community re-integration in the form of home visits, access visits and assessmen
* To communicate effectively, verbally and through manual therapy, with patients to maximise rehabilitation potential.
* Communication skills of persuasion, motivation, explanation and gaining informed consent will be used with a variety of patients. To assess capacity, gain valid consent and to work within a legal framework with patients who lack the capacity to consent to treatment. Barriers to effective communication will regularly be evident e.g. loss of hearing, altered perception, expressive and receptive dysphasia, visual and mental disabilities, depression, pain and fear.
* To use written information to reinforce understanding of the patient's condition and their involvement in the treatment process.
* To liaise with medical, nursing staff and other health care professionals regarding patient care.
* To liaise timeously through multi-disciplinary team meetings, one-on-one training sessions, emails and verbal handovers to advise patients, carers and relatives regarding ongoing patient care.
* To liaise with members of the multidisciplinary team and attend MDT meetings and handovers.
* To regularly update care plans, moving and handling plans and handover to members of the team.
