JustPhysio are looking for a HCPC registered Band 6 Acute Physiotherapist to work for a Hospital in County Durham.

Pay: £23 - £25 (Umbrella)
Position: Acute
Must be HCPC registered
Start Date of Placement: ASAP
End Date of Placement: Ongoing
Working Pattern: 37.5 hpw / M-F / 8-4

Job Purpose:
This post is within the Physiotherapy Team and is supervised by the Clinical Team Lead. The
post holder will be responsible for their own caseload and the supervision of Band 5 staff,
Specialised Therapy Assistants as well as students.
It will be expected that the post holder will take a lead in developing excellence in the
provision of Evidence Based Therapies/Interventions to people requiring Physiotherapy
Intervention.

The post holder will assess the needs of people throughout the Therapy Service and will
support their safe return to their previous lifestyle choice. Examples of needs based therapy
interventions, as well as non-clinical duties, will include any or all of the following:
-Health promotion/prevention
-Rehabilitation needs identification and liaison with appropriate community services
-Assessment of people's needs within their own homes and the prescription/provision
of appropriate equipment/adaptations to enable their safe return to their lifestyle of
choice.
-Representation at relevant clinical meetings.
-Participate in patient/family choice meetings.
-Liaison to Rapid Response/Intermediate Care/ Social Service and Medical services to
ensure the person returns from hospital to a safe environment or to provide a
functional perspective which will support future decisions for discharge.
-Maintain a focus on achieving and demonstrating overall improvements in health care,
prevention of inappropriate admissions and timely discharges from hospital.
-To take a leading role and be accountable for the facilitation of the 'Discharge to
Assess' pathway including delivering on the 'Trusted Assessor' role in line with
nationally mandated guidelines.
-Coordination of team resources as dictated by annual leave, sickness, escalation etc.
-Maintain a focus on achieving and demonstrating overall improvements in health care,
prevention of inappropriate admissions and timely discharges from hospital.
-To provide clinical leadership/management to other professionals and support the managers
in developing services.
-To provide people with a range of clinically reasoned physiotherapeutic interventions
following their admission to hospital. This will include assessing, through early identification of needs, timely discharge planning and rehabilitating patients to enable their safe return home or to ensure they are placed in a safe environment where continued rehabilitation/care is provided. This will be achieved through improved integrated discharge pathways and working collaboratively with discharge liaison and the patient flow teams.
