Our lovely client in Wales is looking for a Physiotherapist to join their team .

Purpose of the Role
To provide a physiotherapy service to service users within our clients' homes, undertaking all aspects of clinical duties as an autonomous practitioner with responsibility for a patient caseload working without direct supervision.

Main Responsibilities:
*Responsible for the assessment, treatment and management of patients with complex or difficult presentations that require sensitivity for successful intervention. To show an understanding of risks involved and ensure a high standard of patient care.
*Contribute to the clinical diagnosis and determine appropriate and individualised physiotherapy treatment. These include service users on the general unit, the young physically disabled unit and residents in the unit with advance mental health needs.
*Liaise with all members of the multi-disciplinary team and attend relevant case reviews. Maintain good communication and working arrangements with Primary Care Trusts and other agencies.
*Efficiently manage own clinical caseload within timescales and clinical priority seeking advice and support from senior staff where appropriate.
*To be professionally and legally accountable for all aspects of the work in accordance with the Chartered Physiotherapists Code of Practice.
*Ensure that valid consent is agreed with the patient working within the legal framework. This involves a full explanation of the outcome of the assessment and evaluating the patients understanding of the proposed treatment options and then agreeing treatment plans and goals.
*The post holder must identify potential and actual stressful, upsetting or emotional situations and manage them in an empathetic manner. Communication is frequently with patients who have complex emotional, physical and psychological conditions and the post holder required mental effort and concentration to be responsive to unpredictable patient demands.
*Responsible for all treatment records, correspondence and reports relating to individual patient caseload in accordance with Fieldbay documentation standards, ensuring these are regularly reviewed and revised as appropriate.
*Provide specialised advice, either spontaneous or planned, teaching and instruction to relatives, carers and other professionals, to promote the aims of physiotherapy and to ensure a consistent approach to patient care.
*Undertake measurement and evaluation of work and current practice through audit and outcome measures. Make recommendations for change, as appropriate, and incorporate as necessary into individual work.
*Demonstrate a sound understanding of Clinical Governance and Risk Management
*Undertake any necessary skills training, professional updates and mandatory training as appropriate to the post and as directed.
