JustPhysio are looking to recruit HCPC registered Band 6 Community Discharge Physiotherapist's to work for one of our NHS clients based in the Lancashire region. This role is due to start immediately for a period of 3 months.

Rates: £21-£24 per hour (UMBRELLA)
Hours: Full Time, Mon- Fri
Essential: HCPC Registration

will assess the patient in the care home for therapy needs and functional requirements, care needs and have close liaisons with the social worker in order to agree discharge plans for the patient to be discharged home.

It is essential that applicants have previous system 1 experience and community experience and have a NHS smartcard.
