JustPhysio are looking to recruit HCPC registered Band 7 Community Discharge Physiotherapist's to work for one of our NHS clients based in the London region. This role is due to start immediately for a period of 3 months.

Rates: £24-£28 per hour (UMBRELLA)
Hours: Full Time, Mon- Fri
Essential: HCPC Registration

Requirements:
* Previous community experience.
* Car driver and access to own car for work.
* Non-car drivers would also be considered.
It is essential that applicants have previous system 1 experience and community experience and have a NHS smartcard.

Role specification:
* Rolling contract to cover hospital discharge service triage and clinical case load
* 37.5 hours week, 8am-4pm or 12pm-8pm shifts
* Opportunity for some weekend shifts and to support team hours of 8-8.
* Hospital discharge service includes a discharge to assess model. Role includes co-ordinating discharges, completing onward referrals (packages of care, therapy referrals, nursing referrals)
